Request Your Credit Report

Alliant Cooperative Data Solutions, LLC (“Alliant”) is a consumer reporting agency that provides consumer reports to third parties for the purpose of credit risk assessment and/or other purposes as permitted by law. As a consumer reporting agency, we may disclose information about you only to companies that you have authorized to access your information, or are otherwise permitted to access this information.

If you have recently applied for credit, housing, employment or had any other adverse action taken against you based on information provided by Alliant, you are entitled to receive a copy of that same information free of charge. Consumers can request a free copy of their report annually. In addition, we will reinvestigate any information that we maintain which you believe is inaccurate and will correct any such information that is determined to be inaccurate or incomplete. For a copy of your report, please complete a consumer request form and mail the completed form to our office:

Please Note: We cannot inform you of the specific reasons why you may have been denied. We can only supply you with the same information given to the lender at the time of your application.

The Fair Credit Reporting Act (the “FCRA”) allows Alliant to include your name and address on prescreen lists used by creditors or insurers for sending you unsolicited offers of credit or insurance.  If you would like your name and address to be removed from such prescreen lists for 5 years—”beginning 5 business days after the date Alliant receives your request”—please fill out the form. 

To permanently opt-out from such prescreen lists, please download and print the attached form, complete and sign it, and return it to us at the address below:

If you are unable to print the form, you may mail us your written request and include the following information:

  • Your first, middle and last name (include Jr., Sr., III, etc.)
  • Your current address
  • Your previous address (if you have moved in the last 6 months)
  • Photocopy of your government issued ID or copy of bill mailed to your address
  • Your date of birth
  • A statement whereby you request the permanent removal of your name from the prescreen lists that Alliant provides to businesses that send firm offers of credit or insurance
  • Your signature

Removing your name from prescreen lists does not affect your ability to apply for or obtain credit or insurance

While your name will be permanently removed from the prescreen lists that Alliant provides to businesses for the purpose of making a firm offer of credit or insurance, you may continue to receive offers from sources that do not use Alliant to compile their prescreen lists.

If you suspect that you have been or are about to become a victim of fraud or a related crime, including identity theft, you may contact the national consumer credit reporting companies to request a fraud alert or other alerts permitted under the Fair Credit Reporting Act be placed in the files such companies maintain on you. If you are a victim of identity theft, you should contact the Federal Trade Commission’s Identity Theft Hotline to obtain more detailed information about how to protect yourself and recover from identity theft and to file an identity theft complaint.

By sharing your identity theft complaint with the FTC, you will provide important information that can help law enforcement officials across the nation track down identity thieves and stop them. The FTC can refer victims’ complaints to other government agencies and companies for further action, as well as investigate companies for violations of laws the agency enforces.
